I am having trouble with my 1989 s10 4.3. It is having trouble starting, it dies immediately after starting, it backfires when running before dieing, and sometimes runs perfectly fine. I have cleaned the throttle body intake and would love suggestions from there. Thanks.

Sounds like you may have a couple of issues here.
I would replace the Ignition module, distributor cap and rotor.
These are common failure parts and can cause the symptoms you have described.
